How do we work with case managers?

We offer four ways to work with case managers, integrated care boards (ICBs), and clients:

Fully managed care service

We recruit and train a dedicated care team tailored to the clients' needs. Each client is supported by a designated Nurse Manager located within 45 minutes, along with a Mobilisation Nurse Manager nationwide, team leaders and field supervisors in every location, which means we have a large backup team who are able to work with our clients at short notice.

We integrate our team members into your existing care plan, with each team member working their desired number of shifts per week. This arrangement allows us to use their availability flexibly to support your needs – especially during your team's holidays, cover shortages, and recruitment phases –ensuring seamless continuity of care for your clients.

Working with clients and their commissioners, we recruit carers specifically to the brief provided. We can provide new members for your team who are trained, have been inducted, and meet all of the relevant employment governance, either by selecting team members from our existing pool or working towards a recruitment deadline.

Superior Healthcare can offer a team of clinically experienced nurse managers to support with the clinical aspects of care for your directly employed team. We can work in several ways, providing care plans, risk assessments and inductions plans, as well as identifying training needs and providing a wide range of clinical training and competency sign-off.

Rehabilitation outcomes

We strive to achieve successful rehabilitation outcomes where possible for clients following a life-altering incident. We take pride in knowing that the quality of our service has led to a reduction of care required by our clients.

One example of a positive outcome involved a client living with a traumatic brain injury. When they were first referred to us they received round-the-clock, double-handed care. However, due to the success of their care package, we were able to reduce their care to one-to-one support.

How do you set up a care package?

When commissioning Superior Healthcare to provide at-home complex care, we will appoint a Nure Manager with the skills and experience to oversee the care package.

It’s important to us to handpick a team of experienced healthcare professionals who can assist our clients get the most out of life and continue doing the things they enjoy.

The level of care is collaboratively planned with everyone involved in a client’s care. We work with case managers and professional partners to create a robust care package, that works for everyone, with the clients’ needs, wants and goals at the centre.

  • We will allocate a Nurse Manager who will oversee the care package.
  • Before providing care, we will agree on the plan for the support that we will provide including defining the clients’ goals and outcomes for the service – which we will use to review and measure the performance of our nurse-led service. Reviews are ongoing but formal reviews are completed quarterly.
  • As well as introducing experienced support workers and/or nurses from our established team to you, we will recruit and train others who match the client’s personality and clinical care needs.
  • The Nurse Manager will guide and oversee the care provided by our complex care-trained healthcare assistants and support workers.

We work with individuals throughout their lives, on a long-term basis. Along the way, our Nurse Manager and expert clinical team will identify changes in the client’s health, needs and personal goals or lifestyle and evolve the care they provide.

What complex care needs do you support?

Our team of specialist carers, support workers, and nurses – including paediatric nurses – support people with conditions such as:

  • Acquired brain injury
  • Traumatic brain injury

What client-specific training do you offer?

Nurse managers identify client-specific training requirements at the care needs assessment and work with our in-house training and development team to deliver specialist, client-specific training.

We train teams of carers, support workers, registered nurses and children’s nurses, to support children, young people, and adults living with ongoing conditions, such as people who:

  • Have a tracheostomy and/or ventilator (BiPAP/CPAP) and require support to maintain a clear airway
  • Require seizure management and rescue medication such as Buccal Midazolam (and Rectal Diazepam if required)
  • Use a suction or cough assist device
  • Need support to receive medication, nutrition, and hydration via gastrostomy tube
  • Require assistance with personal care including continence care
  • Need assistance with medication administration
  • Require bowel management following a spinal injury
  • Need positive behaviour support to live safely

Renowned across the South East, our training programmes are extended to private hospitals, hospices, and care settings.

How do you recruit and retain your care team?

Superior Healthcare employs a team of trained carers, support workers and nurses including children’s nurses who are recruited to meet our clients’ individual needs.  We complete stringent pre-employment checks as well as providing comprehensive induction and ongoing training:

  • We apply for Enhanced DBS checks for all members of our care team and complete annual update checks via the DBS Update Service.
  • We verify employees’ complete career history and obtain employer references covering at least the past three years’ employment.
  • We provide comprehensive induction programmes which are delivered in our classroom locations and facilitated by our team of experienced and qualified trainers.
  • Our team members take part in ongoing training and continuing professional development opportunities, including Level 2/3 Diplomas in Health and Social Care.
  • We proactively support employees – every member of the Superior Healthcare team has access to a comprehensive Employee Assistance Programme, with additional wellbeing support offered by our in-house wellbeing team.
